Guideline Summary: Chronic limb-threatening ischemia (CLTI) is associated with mortality, amputation, and impaired quality of life. These Global Vascular Guidelines (GVG) are focused on definition, evaluation, and management of CLTI with the goals of improving evidence-based care and highlighting critical research needs.

The spectrum ranges from asymptomatic or intermittent claudication to necrosis and limb loss. A sudden decrease in limb perfusion that threatens limb viability defines acute limb ischemia (ALI) and represents a major vascular emergency. The clinical presentation is considered to be acute if it occurs within 14 days after symptom onset [ 2, 3 ]. To pass the examination a Candidate must achieve all of the following: 1. a total score of at least 168 out of the maximum possible 224; 2. pass at least two of the three open technical assessments, and two of the three endovascular sections; 3. to pass at least 4 of the 5 sections.

CVIR Endovascular 3 (1) DOI: 10.1186 The European Society for Vascular Surgery (ESVS) has developed guidelines for treating patients with acute limb ischaemia (ALI). The focus on the guidelines is on lower limb acute ischaemia; Revascularization is the mainstay of therapy in patients with CLI, in the U.S. guidelines, or chronic limb-threatening ischemia, in the European guidelines . The ESC/ESVS guidelines endorse the WIFI classification system to evaluate for the risk of amputation, which gives scores based on Wound, Ischemia, and Foot Infection . The American document suggests that there are no definitive clinical classification systems for PAD and believes this is an evidence gap that needs more investigation. SVS/ESVS/WFVS Guidelines on Chronic Limb-Threatening Ischemia Clinical guidelines on chronic limb-threatening ischemia were released in June 2019 by the Society for Vascular Surgery (SVS), European Patients with acute limb ischemia (ALI) should be emergently evaluated by a clinician experienced in assessing limb viability and revascularization techniques.
